If you would like to set up a Service Contract with us please read the contract agreement here, and fill out the form below with your information and we will contact you by the end of the day or the next morning for requests submitted after 4:30pm MST, Monday - Friday.
If you have a current Service Contract directly with Quilt-EZ or are under warranty please fill out the form below and we will contact you in the order your request is received. Please expect contact by the end of the day, or the next morning if your request is submitted after office hours; Monday - Friday, 8am-5pm MST.
